Community Support Services
Chafee Program Purposes
- To ensure that youth are provided education, training, financial support, community linkage and other needed services.
- To support and serve youth to complement their efforts toward self-sufficiency.
- To assure that youth make the transition from foster care to independence successfully.
Project Services
Chafee Community Support Services may provide you free assistance in housing, medical care, education and employment as well as in emergency situations.
We have field staff who can meet with you locally. For more information call 866-720-3605.
Project Eligibility
Youth may be eligible if they are likely to remain in foster care until 18 and have not reached their 21st birthday; they are former foster care recipients, who age out of foster care at 18 or older and have not reached their 21st birthday.
Project Referral Protocol
E-mail Chafee@hsc.wvu.edu with:
- Youth’s Name
- DHHR Worker Name and Phone Number
- Referent Name and Phone Number
- Expected Date of Discharge from the Foster Care System
